Unrepresented patients—people without capacity to make medical decisions who also lack a surrogate decision‐maker—form a large and vulnerable population within the United States health care system. The burden of unrepresentedness has rightly prompted widespread calls for more and better advance care planning, in which still‐healthy patients are encouraged to designate a surrogate decision‐maker and thus avoid the risk of becoming unrepresented. However, we observe that some patients, even with available social contacts and access to adequate advance care planning services, simply (...) decline to name a surrogate decision‐maker. We propose a novel concept of “informed refusal of representation” (“IRR”) to characterize the position held by some such patients, who are often overlooked in prior work on unrepresentedness. We then discuss physicians’ ethical obligations in the face of such a refusal and avenues by which physicians can support patients without surrogates in receiving goal‐concordant care. (shrink)

Genetic variation in fitness is the fundamental prerequisite for adaptive evolutionary change. If there is no variation in survival and reproduction or if this variation has no genetic basis, then the composition of a population will not evolve over time. Consequently, the factors influencing genetic variation in fitness have received close attention from evolutionary biologists. One key factor is the mode of reproduction. Indeed, it has long been thought that sex enhances fitness variation and that this explains the ubiquity of (...) sexual reproduction among eukaryotes. Nevertheless, theoretical studies have demonstrated that sex need not always increase genetic variation in fitness. In particular, if fitness interactions among beneficial alleles (epistasis) are positive, sex can reduce genetic variance in fitness. Empirical data have been sorely needed to settle the issue of whether sex does enhance fitness variation. A recent flurry of studies1-4 has demonstrated that sex and recombination do dramatically increase genetic variation in fitness and consequently the rate of adaptive evolution. Interpreted in light of evolutionary theory, these studies rule out positive in these experiments epistasis as a major source of genetic associations. One challenge for those reading methodological debates in low consensus fields is determining the outcome when participants do not share standards. When parties to a debate do not agree on the standards to be used in assessing their arguments (i.e., quality), it may be useful to ask first if parties’ contributions meet their own expectations (i.e., integrity). Most protocols for review of qualitative research specify some form of quality assessment. These protocols normally require some test of internal coherence. Coherence is (...) also relevant when describing the match between a rebuttal and the argument it answers. In 2019,Nursing Inquirypublished a critique and rebuttal of the methods used by the Joanna Briggs Institute. In this essay, we attempted to use the Joanna Briggs Institute's own quality assessment standards to assess their rebuttal of this fundamental critique. We found it possible to use the Joanna Briggs Institute's own quality assessment standards to assess this rebuttal, and we found that JBI's rebuttal did not meet their own standards. (shrink)

Peter Kwasniewski shows that St. Thomas contemplates the nature of ecstasy at key stages in the development of his thought and that it plays a crucial role in his doctrine of love. St. Thomas recognized that all love involves ecstatic transcendence, whether it be the creature's self-oblation to the Creator, the reverence of an inferior for a superior, a superior's generosity toward an inferior, or the mutual affection and help of equals joined in friendship. Love of persons for (...) their own sake generates an ecstatic love in which the self is borne as a gift to another subject by sharing a common life aspiring to common goods. This stark formulation of the animalist slogan distances it from nearby claims—that we are essentially animals, for example, or that we have purely biological criteria of identity over time. Is the animalist slogan—unburdened by modal or criterial commitments—still interesting, though? Or has it lost its bite? We suggest that in the particular context of medical education, ethics can be considered in a similar way to other kinds of knowledge that are categorised and shaped by academics in the context of wider society. Moreover, the study of medical ethics education is translational in a manner loosely analogous to the study of medical education as adjunct to translational medicine. Some have suggested there is merit in the idea that much as translational research attempts to connect the laboratory scientist's (...) work to its implications for patient care, translational ethics focuses on bringing ethics scholarship into the sphere of personal and public action. We distinguish the term ‘translational ethics’ (the study of ethics being translated between academy, classroom and clinic) from other prominent definitions in the bioethics literature. To do this, we build off a notion of knowledge translation that focuses on the nonlinear movement of information that comes to professionals through multiple competing sources. We suggest that this knowledge, and particularly knowledge about ethics, becomes embodied by the individual. It is through a reflective practice that internally embedded ethics knowledge might be modified, and this work might be best carried out with a moral community that maintains a sense of practical wisdom. Applying this translational approach to the study of medical ethics education can be both academically relevant and practically useful. This view of translation can help bridge the evident, multidirectional relationships between research, education and performance. It might also create further opportunities to develop medical ethics education theory. (shrink)

This volume contains translations of Xenophon's Apology of Socrates, Oeconomicus, and Symposium, accompanied by illuminating critical essays. In his introduction, Bartlett notes that "after a century and a half of neglect, stemming from indifference or outright contempt, the writings of Xenophon are once again attracting serious scholarly study". These three sets of text and commentary will convince any student of ancient philosophy that Xenophon's masterful dialogues deserve a higher place than they often receive. It is not surprising that Cicero, Quintillian, (...) and Plutarch among the ancients, and Edward Gibbon among the moderns, found such profit in his works, for Xenophon is a daring rhetorician and a brilliant exponent of Socratic method. Many earlier translators, resting on prejudice and indulging in literary affectation, have done great disservice to our appreciation of Xenophon. The translations offered in this new volume dispel at once all such impediments. Here we have conversations characterized by finesse, wit, and spiritedness, qualities that emerge simply by letting the author's vigorous rhetorical style speak for itself in English that mirrors the Greek as closely as possible. In this book, Goodman has made a major contribution to the study of the social and political currents of the French Enlightenment. Previous histories of the period tended to gloss over, or ignore downright, some of the most important people and institutions involved in the gradual extension of literacy and public debate that would culminate in the upheavals of the French Revolution. In particular, the central role of the Parisian salon and the work of its presiding genius, the salonnière, have (...) not received the attention they merit. It is Goodman's thesis that the salon was chief home to the adventuresome forays of the philosophes, providing a fertile context for the development and communication of ideas essential to the eighteenth-century vision of encyclopedic and humanistic learning. Without the weekly salon meetings, without the tireless epistolary and conversational efforts of their female governors, the "work of enlightenment" would not have materialized, let alone evolve as it did. A research project by the Schools of Theology and Psychology of Australia’s Charles Sturt University surveyed a large sample of Salvation Army officers. This article considers survey responses to two questions relating to end-of-life care: the use of pain medications that may shorten life, and the cessation of fluid and food intake. The results of the analyses are evaluated in terms of Michael Banner’s proposal that moral theology should more assiduously converse with ‘patient ethnographic study’, which the survey instantiates to (...) some extent. Banner’s proposal and the results of the survey are contrasted to Peter Singer’s analytical moral philosophical dictums on end-of-life care. The results are also compared to a metastudy by Andrea Rodríguez-Prat and Evert van Leeuwen of 14 ethnographic studies of those who wish to hasten death at the end of life. We conclude that respondents exemplify a form of moral reasoning that is embedded within Christian spirituality; counters the assumptions of Singer’s approach; contrasts the diminishment of ‘meaning’ at the end of life, as seen in Rodríguez-Prat and van Leeuwen; and deserves further respectful ethnographic study. Kierkegaard wrote much on the subject of love, but few of his treatments are as eloquent and penetrating as this series of "disputations" built around texts of sacred scripture. His lucid analysis of the parallelism and divergency of worldly and Christian love make this book a great classic in the subject and a fine place to begin reading Kierkegaard if one has not had the pleasure of his acquaintance. As with other works of the author, one finds here the same (...) unique combination of impassioned argument, rapt storytelling, and flashes of wisdom. It is often impossible to say whether Kierkegaard is speaking philosophically, theologically, poetically, or autobiographically. A large part of the mesmerizing beauty of his prose comes from the interchange of earnest exegesis and lyrical digression. (shrink)

Video ethics in educational research involving children is a recent topic that has arisen since the increase in the use of visual mediums in research especially with the development of new and ubiquitous internet technologies and social media. This paper emerged as an expressed concerned by a group of scholars associated with the new Video Journal of Education and Pedagogy that was established in 2016. The paper is the result of a collective writing process over a period of a few (...) months that discusses visual studies in education and visual ethics in relation to qualitative research in education, and as it applies to children. The animalist says we are animals. This thesis is commonly understood as the universal generalization that all human persons are human animals. This article proposes an alternative: the thesis is a generic that admits of exceptions. We defend the resulting view, which we call ‘generic animalism’, and show its aptitude for diagnosing the limits of eight case-based objections to animalism.

Free rides and observational advantages occur in visualizations when they reveal facts that must be inferred from an alternative representation. Understanding whether these concepts correspond to cognitive advantages is important: do they facilitate information extraction, saving the ‘deductive cost’ of making inferences? This paper presents the first evaluations of free rides and observational advantages in visualizations of sets compared to text. We found that, for Euler and linear diagrams, free rides and observational advantages yielded significant improvements in task performance. For (...) Venn diagrams, whilst their observational advantages yielded significant performance benefits over text, this was not universally true for free rides. The consequences are two-fold: more research is needed to establish when free rides are beneficial, and the results suggest that observational advantages better explain the cognitive advantages of diagrams over text. A take-away message is that visualizations with observational advantages are likely to be cognitively advantageous over competing representations. (shrink)

“I dare to guarantee [the reader] that from this book he will acquire a clarity about and a deft drilling in individual dogmatic concepts that usually are perhaps not so easily obtained”, writes Kierkegaard at the beginning of this fascinating inquiry into the character of Adolph Peter Adler, a rural pastor and advocate of Hegelian philosophy who asserted in 1843 that he had received a special revelation from Christ but later contradicted himself in a flurry of intellectual prevarication. As (...) Kierkegaard notes, the supposed revelation looks suspiciously like Hegelian metaphysics confusedly combined with half-understood Christian concepts, and the “clarifications” Adler later sent to an ecclesiastical investigator are “evasions”, a way of changing essentially what he had earlier reported. Adler’s ever-changing story about who or what he is, his comical back-and-forth between the incompatible roles of apostle and man of letters, reveals a rootless soul that does not know itself: “he is a private, confused lyrical genius who casually exegetes incidental Bible verses ‘accordingly as these appeal to him’”. Having laid claim to a revelation, however, Adler must be called to account: either he must return to this revelation-fact with the simplicity and fervor of faith, or he must renounce his pretensions and revoke his original statement. For many years Kierkegaard labored over different versions of the book before he abandoned plans for its publication. One can understand his hesitancy. The finished product demolishes Adler’s position with the fury of a worked-up pugilist who keeps hitting his opponent after the knock-out bell has rung. Kierkegaard felt sorry for Adler and feared that his withering critique would “have too strong an effect on him”. (shrink)

This collection of texts on the Sublime provides the historical context for the foundation and discussion of one of the most important aesthetic debates of the Enlightenment. The significance of the Sublime in the eighteenth century ranged across a number of fields - literary criticism, empirical psychology, political economy, connoisseurship, landscape design and aesthetics, painting and the fine arts, and moral philosophy - and has continued to animate aesthetic and theoretical debates to this day. However, the unavailability of many of (...) the crucial texts of the founding tradition has resulted in a conception of the Sublime often limited to the definitions of its most famous theorist Edmund Burke. Andrew Ashfield and Peter de Bolla's anthology, which includes an introduction and notes to each entry, offers students and scholars ready access to a much deeper and more complex tradition of writings on the Sublime, many of them never before printed in modern editions. This paper argues that Christopher Winch's contribution to the debate on the aims of education contains some significant errors and omissions. His definition of work is problematic and leads to the conclusion that education should be directed towards very narrow vocational targets. His argument makes unstated and contestable assumptions about the source of educational aims. Lastly, he underplays the implications of the economic aims of education for the achievement of liberal aims. His programme would lead to less pluralism than the (...) liberal alternative. (shrink)

Among the interlacing series of Kierkegaard’s pseudonymous and acknowledged works published during his lifetime, the various sets of Christian discourses occupy a decisive place. These two volumes in particular share a close thematic relationship extending to the very topics written on—for example, the lessons taught by the birds of the air and the lilies of the field, or the right dispositions for receiving holy communion. Readers acquainted only with Kierkegaard’s more widely-read books, like Sickness unto Death, Philosophical Fragments, and Either/or, (...) would do well to read his explicitly Christian writings if they wish to gain a better sense of the proper context and foundations of his thought. Looking back over his works in 1849, he discerned a progression “from ‘the poet,’ from the esthetic—from the philosopher, from the speculative—to the intimation of the most inward interpretation of the essentially Christian”. The paper emerges from an Editors' Collective, a small New Zealand-based organisation comprised of editors and reviewers of academic journals mostly in the fields of education and philosophy.
